Summary

The aim of this workshop is to share a model that will improve cross-agency collaboration and enable the public to better access State Service agencies.

Co-ordinated Case Management supports working together across the State Services. The Ministry has tested this process and has applied the Co-ordinated Case Management model to several situations within the Ministry resulting in positive outcomes. They have identified key learnings for agencies to recognise and take into account, should the Co-ordinated Case Management model be appropriate to apply within other organisations.

Co-ordinated Case Management is a flexible model that generally requires few additional resources. It can be applied to a variety of situations within different organisations and can improve the coordination of services by allowing a ‘no wrong door’ approach.

The workshop shows what Co-ordinated Case Management looks like in practice using a family / whanau approach.
